NGGC OUTING PERMISSION FORM

Dear Parent/Guardian

On Wednesday April 13th, 2022, the New Genesis Gospel Chorale will be going on a local outing to Activate Canada in Burlington, ON. We have invited your child to attend  with us for a day of fun and connection as they have been apart of one of our projects over the course of the pandemic.

Please read the information below and sign to give us permission to take your child(ren) under our care for the duration of this outing. All fees for the activity, for supper, and transportation will be covered during the trip by NGGC.

Your child(ren) should carry with them comfortable clothing (sneakers, jeans, etc) as there will be some physical activity involved.
